US Navy’s ‘lack of operational oversight’ behind 2017’s Singapore Strait collision

An absence of operational oversight resulted in insufficient training and inadequate bridge operating procedures, a National Transportation and Safety Board report has concluded. It was also found that the master of Alnic MC, the vessel that ran into the warship, could not have reasonably determined that his vessel was in extremis before it was too late to avoid the accident
